12 weeks of foundation-first breakin’ instruction with instructor Rylei, August 19 – November 4. Cultural grounding, real vocabulary, real history. No drop-ins — full series commitment only. Week 12 is the showcase.

Liability Waiver

1. Assumption of Risk. I understand that dance activities involve physical activity and inherent risk of injury. I voluntarily assume all risks associated with participation in Danzversity programs.

2. Waiver and Release. I release Danzversity LLC, its owners, instructors, and staff from any and all liability for injury, loss, or damage arising from participation.

3. Medical Consent. In the event of a medical emergency, I authorize Danzversity staff to seek emergency medical treatment on behalf of my child.

4. Health Status. I confirm that my child is in good health and physically capable of participating in dance activities unless otherwise noted above.

5. Media Release. I grant Danzversity permission to photograph and record my child’s participation for promotional use unless I submit a written opt-out.

6. Payment Policy. The Breakin’ Series is $360 for the full 12-week program, payable as a one-time payment or as 3 monthly installments of $120 (first installment at registration). The installment plan is a payment plan for the full series, not a month-to-month membership — all 3 payments are owed. All sales are final per the Danzversity Refund Policy.

7. Acknowledgment. I have read and understand this agreement and sign it voluntarily.
