YOUTH ENROLLMENT

1. Assumption of Risk. I understand that dance activities involve physical activity and inherent risk of injury. I voluntarily assume all risks associated with participation in Danzversity programs.

2. Waiver and Release. I release Danzversity LLC, its owners, instructors, and staff from any and all liability for injury, loss, or damage arising from participation in programs.

3. Indemnification. I agree to indemnify and hold harmless Danzversity LLC from any claims arising from my or my child’s participation.

4. Medical Consent. In the event of a medical emergency, I authorize Danzversity staff to seek emergency medical treatment on behalf of my child.

5. Health Status. I confirm that my child is in good health and physically capable of participating in dance activities unless otherwise noted above.

6. Media Release. I grant Danzversity permission to photograph and record my child’s participation for promotional use unless I submit a written opt-out.

7. Payment Policy. I understand that all sales are final per the Danzversity Refund Policy. Monthly subscriptions auto-renew until cancelled.

8. Acknowledgment. I have read and understand this agreement and sign it voluntarily.
