1. Assumption of Risk. I understand that dance activities involve physical activity and inherent risk of injury, and I am the parent or legal guardian (or their authorized caregiver) of the child named above.

2. Waiver and Release. I release Danzversity LLC from any liability for injury arising from my child’s or my own participation.

3. Supervision. A grown-up stays on the floor with the child for the full class.

4. Payment. Drop-in fees are for the dates selected and are non-refundable. All sales are final.
